    During the on site inspection, counselor observed that provider did not document fully the fire drill. Fire drills T/A: A current attendance record and parent contact information must accompany child care personnel out of the building during a drill or actual evacuation and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill.     During the on-site inspection, counselor observed that no monthly checks were conducted on fence base, as some of the support beams and fence slats have rotted, and a portion of the fence is no longer securely anchored. Outdoor Equipment All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts.
