    During the onsite inspection, counselor noted that parents were not provided information on distracted driving for the month of September. T/A: Enrollment Information Annually, in the months of April and September, the child care facility must provide parents/guardians of the children enrolled in the facility with information regarding the potential for distracted adults to fail to drop off a child at the facility and instead leave them in the adults vehicle upon arrival at the adults destination.CF/PI 175- 12, brochure, which is incorporated by reference in 65C-22.001(7)(x), F.A.C. and may be obtained from the Departments website at www.myflfamilies.com . Corrected, provider is aware of the distracted driver enrollment requirements, and will provide parents with required information in the months of April and September. Next parent notification is due in April. The influenza virus parent notifications were monitored during the inspection

    During the vehicle inspection, counselor observed that the transportation log did not include all required elements. T/A: Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). Corrected CD and staff are aware of the requirements for transportation and will along with their physical sweep of the vehicle, also initial to verify the transportation log is accurate
