Building 2 classroom had blankets stacked with the cots. Blankets are normally stored in plastic bags in between the cots. Staff will wash linens before next use. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. All bedding and linens must be thoroughly cleaned and sanitized at least once a week, and before use by another child

Director notified Child Care Regulation that a child was left outside on the playground for a few minutes. Teacher quickly went outside and brought child back inside. Director met with each teacher individually after the incident. Director will reteach supervision to all staff at the next staff meeting. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised

Child care personnel was observed not following the center's discipline policy. When the child was brought back into the classroom, he fell to his knees and staff picked him up by the arm.
