During the inspection, the Licensing Specialist observed an employment history verification was not documented and on file. The Operator has been given until 7/17/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, FCCH/LFCCH Handbook, Section 4.1, E: Initial Screening: Operators, household members (adults and children 12 to 17 years of age), substitutes, volunteers and Large Family Child Care Home employees must have a level 2 background screening clearance from the Department prior to obtaining a license, residing in the home, employment, or volunteering unsupervised with children. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. E. The employer/owner/operator must conduct employment history checks for substitutes, including documented attempts to contact each employer that employed the individual within the preceding five years and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained. P. Documentation/clearance for the initial screening must be included in the Departments licensing file. Screening information for the employee and household members must be documented on CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in 65C-22.001(7)(b), F.A.C. During the inspection, the Licensing Specialist observed the substitute, and employee did not complete the ten hours of annual in-service during the 12-month licensure period. The substitute is missing 10 hours in-service training. The employee is missing 1 hours of annual in- service training. During the inspection, the Licensing Specialist observed during the licensure year, the operator did not conduct monthly fire drills utilizing the approved fire alarm system or smoke detector at various dates and times when children are in care in that the Operator did not have a record of having done any fire drills. During the previous inspection on 12/16/2025, a fire drill was conducted in the presence of the Licensing Specialist. This standard has been brought back into compliance in that a Fire Drill was completed in the presence of the Licensing Specialist during the inspection. There were 3 adults and 5 children present. The Operator used the secondary smoke alarm to complete the fire drill due to the smoke detector closest to the classroom not working. During the inspection, the Licensing Specialist provided technical assistance, FCCH/LFCCH Handbook, Section 7.21, A: Fire Drills A. During the homes licensure year, fire drills utilizing the smoke detector(s) must be conducted monthly at various dates and times when children are in care. *******Lockdown and inclement weather drills shall be conducted a minimum of one time each per operating year when children are in care and the documentation must be maintained for two years. A lockdown or inclement weather drill may substitute for one monthly fire drill. No more than three fire drills may be substituted during a 12-month period. ******* Home emergency preparedness plan drill records showing the type of drill, date conducted, num-ber of children in attendance, and time taken for all individuals to complete the drill will be docu-mented by the Operator for all the drills and maintained for one year.
