25-04 · Technical assistance was provided to facility staff regarding proper perimeter security and gate maintenance requirements. The violation was corrected on-site when it was determined that the bolt of the gate was not holding properly and could not stay shut. The facility replaced the defective bolt and the gate can now remain securely closed. The facility shall ensure that all gates providing access to unprotected areas remain securely closed and latched at all times, particularly when children are present in outdoor play areas, to maintain proper perimeter security and prevent unauthorized access or child egress from the facility
