According to staff K.H: The incident happened during Christmas Holiday, it was herself, Ms. Donna, and Nicole with the children on the play area. Ms. Donna asked her to go fix the Christmas tree lights because they were not working as they needed to take picture by the Christmas tree. Coming from the kitchenette area, she did not know who came inside first, she saw Ms. Donna and Ms. Nicole in the room leaving the children alone on the playground. She was mad, she left everything and headed outside. On her way to playground, Ms. donna stopped her and asked if she could please go outside, there was no one with the children on the playground

According to staff K.H: Nicole used profanity words in front of the children. The child F. had issue and was in the restroom; Ms. Nicole went to say, did shit in your pant today. Ms. Christy who was a teacher at the time, told Ms. Nicole "he is a kid you can not this word" Ms. Donna was present but acting like she did not hear anything. Nicole threatened to hit two different children on two different occasions. First time was in the elevator and the second time was in the classroom. The elevator incident: Child N.B did play a bit aggressive with the other children. They were in the elevator going to the music room in second floor, child N.B was going after another kid. Ms. Nicole put her hand up like she would swat her hand away and then she stopped, she did not touch the kid . The classroom incident. Child K.K pushed another kid in the classroom, Nicole did the same thing, but she did not make any contact with the child

According to staff K.H: Nicole as a sensory bin (rice and beans) that she brought from home. She kept it in the cubbys holes in the classroom which is accessible to children. The sensory bin had a cover, but it was not a locked cover, children could be observed playing with it. Rice and beans without supervision, shall be considered as hazardous materials. There were concerns of children putting rices in their noses, ears, mouths,.......... . possible slip/fall should they being thrown on the floor: The sensory bin was not observed in the classroom at the time of the inspection
