Counselor was informed an infant on site did not have a file as the parent did not return any of the papers. The director called the mother to complete all documents while counselor was on site. Due date: completed at time of inspection

03-01 · Ratio Sufficient
When counselor arrived at the facility , counselor observed that there was 1 staff with 8 infants in the infant room. The director reported that they actually had more than enough staff present and that the other staff member who is suppose to be in the infant room had stepped out to go to the restroom and the ratio was actually 2-8. Counselor informed the director that when a teacher needs to leave a room for any purpose that they need to call her or another staff member to relieve them so that the room can remain in ratio. The 2nd staff member arrived back to the room, bringing the infant room back into ratio .This was completed at the time of inspection

04-01 · Supervision
On 11/10/2023 around 10:45 AM. As one of the one year old teachers exited the classroom to grab something, a child crawled from by the table, where she was snacking and tried to follow the staff out of the door. The other teacher in the room noticed the infant's movements toward the closing door and tried to catch the door from closing, but was unsuccessful. The child's finger was accidentally caught in the inner door jam of the door. The teacher that was exiting the room heard the child scream scream and opened the door. Both teachers ran to the child and observed that her hand was bleeding. One teacher picked the child up and started administering first aide by cleaning the area to see injury then applying pressure to the open wound until emergency services arrived. The assistant director was notified of incident immediately and 911 and the child's parents were contacted next. The door closed on the child's finger, dislodging fingertip, resulting in an injury that required emergency services to respond. The child was transfer by air travel to Arnold Palmer Children's Hospital

23-05 · Fire Drills
The facilities fire extinguisher's were found to have been out of date since 03/2023. The facility did change companies for the extinguishers The director did send a photo that the fire extinguishers had been recharged shortly after counselor left the facility. The Extinguishers are now serviced through J and E extinguishers and is good through May 2024

A child was left on the daycare van for approximately 20 minutes. The child was discovered by his own mother who came to the center to pick him up. The child was unharmed due to the incident . Completed at the time of inspection

Upon the owner /director arriving back to the center with the school age children, a second sweep of the van was not completed. The owner /director alleged that she had marked on the transport log that the child had gotten off the van when in fact he had not. The staff that is usually in charge of doing a second sweep of the van was checking in a food vendor and the owner /director did not ask the staff to do a second sweep

During the inspection , a walk through of the facility was completed to determine ratio's . During the walk through the counselor observed that the teacher in the one year old class did not assist the child in washing their hands after a diaper change. Completed at the time of inspection

The Daycare owner , Director failed to report the incident of leaving a child on her daycare van to the Florida Abuse Hotline . The incident was reported by an outside party. The owner /director never did report the incident to the hotline even after she was told that she was mandated to do so
