A biting incident on a 2 year old child occurred on 5/9/23. No incident report was completed and documented as required. Technical assistance was rendered 6/23/23 and 6/28/23 as Director understands the importance of incident reporting even if it is the next day. Child is no longer enrolled or in attendance at facility. 6.4 Accident/Incident Notification page 55 A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur

20-06 · Crib Requirements
There was an infant sleeping in a crib while swaddled. The swaddle was removed while counselor was present. The Child Care Facility Handbook states on page 26: A. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. B. When napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician. Written documentation of a physicians authorization must be on file at the facility. Documentation must include the childs name, childs date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid

24-23 · Food Preparation Area
The refrigerator in the four-year-old room did not have a thermometer. One was placed in the refrigerator while counselor was present. The Child Care Facility Handbook states on page 31: Refrigerators/freezers: 1. An accurate thermometer designed to measure cold storage temperature must be placed inside each refrigeration and freezer unit. Thermometers in refrigerators must show a reading of 41 degrees Fahrenheit or below, and thermometers in freezers must show a reading of 0 degrees Fahrenheit or below. The thermometer must be located in the center of the unit and be readily accessible. Thermometer temperature readings higher than specified above require further temperature testing of food samples stored in the unit using a probe type thermometer; and adjustments to the unit setting to reach and maintain the required readings must be made

There were two bottles in an infant room that were not labeled with child's first and last name. Bottles were labeled while counselor was present. The Child Care Facility Handbook states on page 35: Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily

32-07 · Outdoor Equipment
All areas of the playground need mulch to be replenished in order to meet fall zone requirements. Director stated that the mulch is due to be completed on Memorial Day Weekend. The Child Care Facility Handbook states on page 38 and 39: D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area
