Invalid CPR training provided for substitute L.B. First aid training for the substitute was submitted. However, handwritten information was observed on the card and there was no CPR information present. Per Operator, the training instructor was unable to print documentation of the training due to software issues. Per training instructor, she was having difficulties with her printer and is in the process of replacing the printer. The training instructor also stated, that she will not be providing any printed cards. Please be advised that instructor based skills assessment for First Aid/CPR is required with accurate documentation. See supplement
