Staff (CC) level 2 screening was not cleared. The staff was hired to work at the facility on 4/27/2026 without current level 2 background screening. The Department Counselor stop by the facility at 3:59- 4:14pm The Department spoke with the Director (MJ) she stated that they did hire a new staff member and that she is currently not here today as of 5/4/2026. Per Ms. Johnson the staff members background clearance is still in process and has been addressed since last week Friday 5/1/2026. The Department Counselor arrived at the facility at 2: 56 PM to conduct a reinspection. As part of the process, a staff-to-child ratio count was conducted. The Department first entered the 2-yrold class to complete a headcount and then proceeded to the infant room. Upon arrival in the infant room, the department counselor observed two teachers present in the infant room: Teacher (BW): At the utensil sink (washing a utensil) with one infant holding onto her leg. Teacher (LR): sitting on the carpet interacting with children. A physical count of the children presents yielded four (4) children. However, a review of the attendance log indicated six 6children were currently checked in on the roster. When questioned regarding the discrepancy, Teacher (LR) stated that six children were present but one already went home. The Department staff advised that attendance logs must be updated immediately upon a childs departure. However, staff insisted that six children were present earlier, but one went home, leaving one child still missing based on the records. The conversation prompted the Department Counselor to look for the missing child. Around 3:02 PM, through the glass window of the door leading to the playground, the Department counselor observed a child sitting alone at a Tiny Tikes picnic table with her arms on her lap. The child smiled and ran toward the Department counselor with her arms open. Teacher BW, who was positioned directly behind the Department Counselor, immediately retrieved the child and escorted her back into the classroom
