33-01 · Training Requirements
One staff person did not complete the 40-hour introductory childcare training requirement. Director stated staff will complete within the next 30 days. The Child Care Facility Handbook states: 4.2 Training Requirements 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of childcare training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Childcare personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. Part I Courses (30 Hours) Childcare personnel must complete all of the following: ? Child Care Facility Rules and Regulations ? Health, Safety and Nutrition ? Identifying and Reporting Child Abuse and Neglect ? Child Growth and Development ? Behavioral Observation and Screening B. Part II Courses (10 Hours) Childcare personnel must also complete 10 hours of the following Part II courses: ? Special Needs Appropriate Practices (10 hours), or ? Understanding Developmentally Appropriate Practices (5 hours) and one of the following courses: 1. Infant and Toddler Appropriate Practices (5 hours) 2. Preschool Appropriate Practices (5 hours) 3. School- Age Appropriate Practices (5 hours)

The facility's temperature was 62 degrees in one room and 64 degrees in another room. Director asked the owner to raise the temperature. Temperature read 69 degrees at the end of this monitoring. A bottle of ODO Ban and a bottle labeled bleach and water were stored under the classroom hand washing sink that was not locked. Director moved the items to a high shelf and stated that she would have the cabinet lock repaired. The Child Care Facility Handbook states: 3.2 Toxic/Hazardous Material/Firearms/Weapons B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times

The rock climber is the school age playground has rust and peeling paint. The swings in the school age playground have "s" hooks that are not closed. The climber on the preschool playground has peeling paint. Director stated these issues will be repaired and completed by June 2, 2023. The Child Care Facility Handbook states: 3.12 Outdoor Equipment B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. Any open containers with water must be emptied immediately after use, i.e. pots, toys, or other equipment that collects water
