39-03 · Accident/ Incident Notification and Documentation
There was inadequate accident/incident reporting. One child was reported by the parent to have had two incidents in which a report was not written up. The child no longer attends the school. The director was given technical assistance regarding incident accident reporting by licensing specialist. Director reviewed the policy with each staff member and had them initial that they understood. In the Child care facility handbook states on page 55 6.4 -Accident/Incident Notification- A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs

During the inspection, two sinks in two separate classrooms were not working. The Child Care Facility Handbook states on page 20 that: 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

that: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The sink and cabinet in the four-year-old room is rotting and needs to be replaced. The molding at the bottom back where this cabinet meets the wall is rotting and needs to be replaced. There is peeling paint in the four-year-old room. The visible plumbing in the two-year- old bathroom is becoming rusty on the outside. All doorways have peeling paint at the door jams. The Child Care Facility Handbook states: 3 Physical Environment Children are much more susceptible to the adverse effects of environmental factors, materials and toxicants. It is imperative that the childcare environment is conducive to the healthy development of children. 3.1 General Health and Safety Requirements A.
