During the inspection licensing noticed that sippy cups in the classrooms were not labeled or did not have first or last names. This was corrected at time of inspection. Provider stated that she would make sure that each teacher had masking tape and marker so teachers or parents can write the childs name on cup every morning. Technical Assistance given from handbook: 3.9.5 Food Handling B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily

32-03 · Outdoor Equipment
During the inspection Licensing Specialist observed a blue metal sit on truck is rusting and the blue paint is peeling off. The blue metal truck is on the one- and two-year-old playground. Technical Assistance given from handbook: 3.12 Outdoor Equipment A. A child care facility must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities. B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. C. All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Corrective Action: Provider must submit pictures via email showing that the blue metal truck has either been painted or been removed from the playground

Child Abuse and Neglect forms had not been updated annually by 7 staff members, 6 were completed at time of inspection. One staff member listed on the confidential page was not at center yesterday to update form. Provider will have employee sign a new Child Abuse and Neglect form and send that form via email to FSC. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel
