04-01 · Supervision
Upon arrival into the infant room, Licensing Specialist observed an infant asleep in a bouncy seat, inside the play pen area. This infant was alone inside the playpen area situated in the center of infant room. There was no teacher inside the play pen area with infant during observation/inspection. 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Corrective Action: Licensing Specialist observed an infant sleeping inside a playpen unattended by staff. Licensing Specialist observed the staff member assigned to the infant leave the classroom. Licensing Specialist advised the Director that child was unattended once staff left the classroom. Director had the staff member to return to the infant classroom from hallway and remove infant from playpen. Infant was placed in a nearby crib in the classroom

13-01 · Toxic Substances, Hazardous Materials and Weapons
Licensing Specialist observed spray bottles in classrooms used for cleaning not labeled with substance contained inside. Director stated that the yellow substance contained in the spray bottles was Lysol, used for cleaning. Child Care Facility Handbook states the following: 3.2 Toxic/Hazardous Material/Firearms/Weapons B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times. Corrective Action: Licensing Specialist had director provide labels on all spray bottles in all classrooms during inspection

31-05 · Indoor Equipment
Licensing Specialist observed a trash can on a crib sheet inside a crib located near the sink area. Licensing Specialist observed the trash can with used diapers and trash inside. Licensing Specialist advised that putting the trash can inside the crib, on top of a crib sheet is unsanitary. Child Care Facility Handbook states the following: 3.11 Indoor Equipment B. Toys, equipment, and furnishings must be safe and maintained in a sanitary condition following a routine schedule of cleaning, sanitizing and disinfecting. These items must be cleaned and sanitized or disinfected immediately or prior to another childs use if exposed to bodily fluids, such as saliva. Facilities must have a written routine schedule for cleaning, sanitizing and disinfecting equipment, materials, furnishings and play areas. This schedule must include items to be cleaned, sanitized or disinfected on a daily or weekly basis. Daily indoor equipment items include but are not limited to: counter/table tops, toys, drinking fountains, floors, diaper pails, toilets, and sinks. Weekly indoor equipment items include, but are not limited to: linens, machine washable cloth toys, play activity centers, dress-up clothes, cribs, mats, and cots. Corrective Action: The trash can was removed out of the crib and moved to the other side of the half wall. Licensing counselor suggested to relocate the trash can outside of the crib and play area or relocate to under the sink cabinet
