39-03 · Accident/ Incident Notification and Documentation
Counselor interviewed the facility Director concerning then allegations. Facility Director stated the parent received an Accident/Incident Notification and Documentation the day after the incident occurred. A copy of the documentation was provided to the counselor. Documentation was dated and signed by the parent on 2/23/2024. Incident occurred on 02/22/2024. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child Child Care Facility Handbook Page | 55 care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. E. In the event of serious injury or death, the incident must immediately be reported to the licensing authority

11-05 · Child Discipline
Incident report was written by a staff member that witnessed a child being pulled, grabbed by one arm and being spoken inappropriately by another staff member. The child had apparently had a toileting issue that was upsetting the teacher. She confronted the child in this behavior, removing him abruptly from circle time while sitting with his classmates on the rug and taken outside the classroom into the hallway to get reprimanded. This was resolved when another staff member did not feel comfortable with the situation and placed the child away from the teacher. Teacher was removed from the building and replaced immediately with another teacher. 2.8 Child Discipline A. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Spanking or any other form of physical punishment is prohibited. F. The following discipline techniques shall be prohibited in the child care facility: 1. 12-09 · Facility Environment
Counselor observed dead roaches in two areas of the facility. In the front desk area dead roaches were visible on a trap and under the desk as well. In the infant classroom one dead roach was visible in the storage closet as well as between the fridge and the permanent cabinetry. The provider will have to thoroughly clean these areas to be in compliance with childcare licensing standard 12-9, by due date. Childcare Facility Handbook 3.1: A: All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. F: Pest control must not take place while rooms are occupied by children. Childcare providers must adopt an integrated pest management program to ensure long-term, environmentally sound pest suppression through a range of practices including pest exclusion, sanitation and clutter control and elimination of conditions that are conductive to pest infestations. The facility has been actively treating the roach infestation. The facility is taking adequate measures by contracting a pest control company, documentation was provided showing monthly pest control from 2/23/2024 to present
