A two-year-old child was left in the restroom unattended for approximately 5- 7 minutes. The child was discovered by his mother, who worked at the facility, and returned to his class. The director required all staff to complete in-service training entitled " Active Supervision at a Glance". The Child Care Facility Handbook states: B. Childcare personnel must be assigned to provide direct supervision to a specific group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location
