11-3: During the inspection, the director advised that teacher 1 was terminated from the facility on 4/16/2025 for falsifying an incident report, for not following the facilitys discipline policy, by putting child 2 in timeout and for redirecting a child forcefully, causing an injury to the child that broke skin. 16-20 · Indoor Floor Space
25 and observed that the incident between teacher 1 and child 2. Child 2 was on the table and Teacher 1 grabbed child 2 and put her in timeout in the library. Teacher 1 put the band aid on child 2 and was talking to child 2 in the library. The director stated that it looked like teacher 1 grabbed child 2 under each of the arms with two hands, picked her up from the table, put her on the ground and grabbed her hand and brought her to the library. The director stated that teacher 2 saw that mark and got child 2 a band aid. The director stated that he saw the marks on child 2, the skin was broken but it was not bleeding. The director advised that teacher 1 was terminated from the facility on 4/16/2025 for falsifying an incident report, for not following the facilitys discipline policy, by putting child 2 in timeout and for redirecting a child forcefully, causing an injury to the child that broke skin. During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 33 employees. During the time of the inspection, the Licensing Specialist monitored 33 of the 33 employee records. REMINDER: In-service training for 2024/2025 fiscal year must be completed before July 1, 2025. 33-6: During the inspection, Licensing Specialist observed 1 child care personnel did not complete the 5 hour early literacy training within 12 months of their child care industry date. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 4.2.2 and 4.5, C Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. 05-04 · Transportation
5-4: During the inspection, Licensing Specialist observed that the facility usings three different forms for the transportation logs (a form called transportation log where the drivers signature, second signature, time and date are logged; a form called manifest where the field destination is listed, the drivers signature, second signature, time and date are logged; and a form called attendance where the childs name is listed and they are check on the vehicle and off the vehicle.) Licensing Specialist observed that there were 2 entries that had the children checked on the vehicle but not off the vehicle. Licensing Specialist observed 6 entries that did not have the second signature. Licensing Specialist observed 6 manifests, however there were no attendance of the children to accompany the form. Licensing Specialist observed a number of entries on the childs attendance, however there was not a transportation log or manifest that accompanied the entries. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.2 A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable)

During the inspection, the director advised that there are currently 126 children enrolled. During the inspection, CCR monitored the children files that their immunization/Physical has expired from during the months of March to June which was 28 of the 126 children's records. CCR observed two children who had an expired immunization form. The director was given until July 31, 2023, to come into compliance with this standard. During the inspection, CCR provided technical assistance, CCF Handbook, Section 7.1 F Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. F.
