39-04 · Accident/ Incident Notification and Documentation
39-4: During the inspection, Licensing Specialist observed that there 19 accident/incident reports that were not signed by the custodial parent/ legal guardian or the authorized pick-up person on the day that the accident/incident occurred. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 6.4, B & D B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. 12-02 · Facility Environment
12-2: During the inspection, Licensing Specialist observed that the facility was not in good repair in that the walls were peeling in the White Tiger room, the Panda room, the Lion room and the Zebra room. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.1, A All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. 17-06 · Outdoor Play Area/Fencing
17-6: During the inspection, Licensing Specialist observed that a section of the black fence on the VPK playground was missing 3 bottom black rails causing a gap that exceeds 3.5 inches. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.5, F The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3.5 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided

23-02 · Fire Drills
The last fire drill was conducted on 4/28/2025. During the inspection, a fire drill was conducted with Licensing Specialist. The fire extinguisher certification(s) are valid until 3/2026. 23-2: During the inspection, Licensing Specialist observed that the annual fire inspection was conducted on 4/28.2025, however there were non-compliances on the inspection. Staff advised that the fire marshal will complete their reinspection on 5/19/2025. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.8.2, A Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. 33-09 · Training Requirements
During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 45 employees. During the time of the inspection, the Licensing Specialist monitored 45 of the 45 employee records. REMINDER: In-service training for 2024/2025 fiscal year must be completed before July 1, 2025. 33-9: During the inspection, Licensing Specialist observed 3 child care personnel that did not complete the safe sleep training within 30 days of their hire date. During the inspection, Licensing Specialist provided technical assistance, Section 4.2.3 All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. 04-01 · Supervision
4-1: During the inspection, Licensing Specialist observed camera footage from 5-9-2024. The video shows child 2 walk over to child 1. Child 1 was sitting on a purple chair. Child 2 was observed touching child 1 inappropriately for about three minutes. Teacher 2 was observed on the sofa engaged with other children. Teacher 2 looked over to the children while child 2 was touching child 1s privates and teacher 2 was observed talking to the two children and then she went back to talking to the other children. Child 1 had his back to teacher 2. The other children were observed to witness the interaction between the child 1 and child 2. The incident was documented on an incident report and the director retrained the staff on where they should be standing or sitting when supervising the children. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 2.4, A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group

33-6: During the inspection, Licensing Specialist observed 1 child care personnel has not completed the 5 Hour Early Literacy training within 12 months of the child care industry date. During the inspection, Licensing Specialist provided technical assistance, Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. 24-09 · Food Preparation Area
24-9: During the inspection, CCR observed that the handwashing sink in the kitchen did not have hot water. During the inspection, CCR provided technical assistance, CCF Handbook, Section 3.9.1, A.7. Food Preparation Area A separate handwashing station with hot running water, a minimum of 100 degrees Fahrenheit. The handwashing station must include a sink with running water and drainage, soap, trash can, and disposable towels or hand-drying machines that are properly installed and maintained. A handwashing sink shall not be used for any purpose other than handwashing. Handwashing stations must include posted signs visible to childcare personnel and children demonstrating proper handwashing technique. Portable sinks may not be used for dishwashing or food preparation. If a portable sink is used for handwashing in the food preparation area, hot water must be provided
