Complaint alleges that a child had a busted lip on t wo separate occasions for unknown reasons and an incident report was not provided. A complaint investigation was conducted on 5/21/2026 and completed on 5/22/2026. After reviewing staff statements and conducting an inspection of the classroom, the child fell asleep while sitting on a chair that was not age appropriate. The child fell off the chair after falling asleep, which resulted in a busted lip. With regards to the first incident, the teacher and assistant director denied having any knowledge of it or any other incident involving a busted lip. Per teachers statement (Ms. X. R), during lunch time, the child (J. E.) was dozing off while sitting on the chair at the table. The teacher explained that she wet the childs face several times to keep the child awake. The child fell asleep on the table, and the child fell on the ground and hit his mount, hurting his top lip on the floor
