Upon further investigation, it was discovered that the childcare personnel only had a job application signed with the date of May 17, 2023, and the provider claimed they were employees at the center. Additionally, the childcare personnel provided conflicting information about their roles at the facility and one of them left after getting a scar. This situation involves misrepresented information, impersonation, or the provision of fraudulent information to the family services counselor. The provider was given technical assistance by explaining that it is a violation to misrepresent information, impersonate, or provide fraudulent information related to the child care facility to a parent/guardian, licensing authority, or law enforcement, as per s. 402.319(1)(f), F.S.
