LS observed toilet seat in the (3-year-old) bathroom to be unattached from the toilet. The toilet seat was observed under a shelf adjacent from the toilet. Technical assistant was provided per CCF handbook " All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months."

04-01 · Technical assistance was provided per CCF handbook "A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. D. At all times, lighting must allow child care personnel to see and supervise children while in care. E. A program is responsible for the supervision of a child until an authorized individual retrieves the child from the program. A child shall not be released to any person other than the person(s) authorized or in the manner authorized in writing by the custodial parent or legal guardians. All individuals authorized to pick up a child must be identified in writing prior to release by the custodial parent or legal guardian to the program, and the program must verify the individual picking up the child is authorized by using a picture form of identification. Each child transported must be dropped at the designated location as agreed upon by the provider and the custodial parent/legal guardian and released to an authorized individual. F. No person shall be an operator, owner, or employee of a child care facility while using or under the influence of narcotics, alcohol, or other drugs that impair a persons ability to provide safe care and supervision. G. In addition to the number of child care personnel required to meet the staff-to- child ratio, one additional adult must attend field trips away from the child care facility to help provide direct supervision. The individual could be a parent volunteer as long as that person is under direct and constant supervision of a screened and trained child care personnel. H. Children must receive supervision and care in accordance with their age and required needs and be accounted for at all times while bathing or toileting. I. When transporting children in a vehicle or on foot, a telephone or other means of instant communication must be available to child care personnel. Cellular phones, two-way radio devices, citizen band radios, and other means of instant communications are acceptable. "
While conducting complaint investigation, it was admitted by HP that while in the process of cleaning up from a mosaic rock activity transitioning to a yarn activity. HP placed the yarn on the table with a cube and cup. In that time, HP was being assisted by children on the floor (during clean-up). It was observed by HP that two children hard the yarn around victim child's neck in a sawing motion. HP directed children to stop and took the yarn. Victim child neck was observed with a friction mark on neck. Provider was able to correct during inspection by removing the accessibility to yarn by children and administering first aid to victim child

04-18 · Technical assistance was given to provider per Florida Statues and CCF handbook: Section 435.06(2)(a) of the Florida Statutes states, An employer may not hire, select, or otherwise allow an employee to have contact with any vulnerable person that would place the employee in a role that requires background screening until the screening process is completed and demonstrates the absence of any grounds for the denial or termination of employment. Section 5.1 of the Child Care Facility Handbook states A screening must be conducted as a condition of employment. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws
During the commencement of complaint investigation, LS observed an unscreened individual left alone with children in the infant's classroom. This was discovered by LS that the individual observed in the infant classroom was unscreened, during review of personnel files. Upon request of personnel files, the owner handed the file of FL to LS which only had the employment application present. FL's name and date-of-birth was search in the Clearinghouse with the result reading "No Match". Provider was questioned on the background screening result of FL to which provider responded, the individual was scheduled for screening, "I know her personnel from church and I thought I had 30-days". Provider was advised that DCF also needs to know FL and that FL is unable to remain at facility until screening is completion of background screening. Provider was able to return to compliance by having FL leave the facility and someone covered infant classroom for ration

20-06 · Technical assistance was given to provider per CCF handbook "All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C- 22.001(7)(v), F.A.C. During the second routine inspection, FSC noticed that the playground required cleaning. At the time of the inspection, the daily log was not completed by the provider. According to section 3.1, A of the CCF Handbook, it is a violation if the play area is not inspected before use. The playground needs to be inspected daily before the children are allowed to use it. 19-02 · Nap/Sleep Space Requirements
During the second routine inspection, FSC observed that the children were not maintaining a minimum distance of 18 inches from the furniture, each other, and one child was even sleeping in the hallway. This is a violation of section 3.6.2, C of the CCF Handbook. In order to comply with the regulations, it is necessary to maintain a minimum distance of 18 inches between each napping/sleeping space. The provider was provided with technical assistance to help ensure that they follow this guideline in the future

During a routine inspection, FSC noted a 5-month-old baby girl inside a crib with a "bep" on her neck and before the that there were items inside the crib that could pose a strangulation or suffocation risk. This is a violation according to Section 3.6.4 of the CCF Handbook. FSC provided technical assistance to the provider by explaining that children cannot have any item inside the crib or around their necks. The "bep" was removed by FSC to eliminate the risk
