A complaint was received stating that on May 13, 2025, while outside on the outdoor play area children were sitting on the bench. One child got off the bench and another child L sat down. The first child returned to the bench and pushed the child off the bench and hit the child L in the head with a rock. A complaint investigation was conducted today, and the director and two-year-old teacher were interviewed. According to the teacher she was on the outdoor play area with six -two-year-olds at 4:50pm on May 13, 2025. She stated that one child (a little girl) was sitting on the bench and when that child moved from the bench L sat down. The teacher stated that the first child return to the bench and pushed L off the bench. She was dealing with another child when L walked over to show her that he had been hit in the head. The teacher stated she turned to the other child (a little girl) who she observed holding a rock in her hand as if she was ready to throw it. The rock was approximately the size of a plum. This complaint is substantiated

A complaint was received that a child was hit in the head with a rock and the facility applied a bandage and cream to the area prior to the arrival of the child's guardian. A complaint investigation was conducted today, and the director was interviewed. Ms. S. stated that the child L was brought into the building after the incident and that the staff stopped the wound from bleeding and then applied a bandage and neomycin antibiotic ointment to the cut on L's head. Facility does not have written authorization from the guardian to apply ointment to any wound on the child. This complaint is substantiated
