Provider self reported that a child was left unattended in the playground around 5:15pm to 5:25pm. The child was not injured. A complaint inspection was conducted on 11/2/2023 and a statement from the director (SD) was received. Per director, transition happens at 5:00pm. The 2y/o teacher (EG) leaves and the new teacher takes over the class (SP) at 5:00PM. Sometime after the transition the child went missing. The new teacher either failed to do the face to name check or looked over the child's name. The parent arrived at 5:25PM to pick up the child and the teacher didn't know where she was. She proceeded to search for her and when she opened the door to check the playground, she found the child playing in the playground by herself. The director believes the child slipped outside when the previous teacher(EG) went to the playground retrieve a cot. Teacher (EG) was already off the clock and may not have noticed that the child followed her into the playground. A lack of supervision class 1 noncompliance and a notice of violation is being issued

During today inspection I observe an insufficient amount of play equipment and materials available in the Toddler classrooms. Technical assistance was provided to ensure sufficient amount of play equipment and materials available for the children
