A complaint allegation was received alleging that the school is only cleaned when inspections are scheduled and carpet is only cleaned 2x per month. A complaint inspection was conducted on 6/4/2024 and a walkthrough of the facility revealed the following items in need of cleaning/removing; - infant toys (no place for mouthed toys) -torn infant carpet -classroom rug needs cleaning: there are particles on the floor that infants crawl through, like pieces of paper and debris - high chair cover and straps A conversation with the Director revealed that the carpets are cleaned every other Saturday but there is no proof. Director stated that the owner is the one that keeps the receipts. TA was provided that carpet throughout the school needs continued, more frequent cleaning to include washing of the rug since children touch the floor when crawling or playing. This complaint is unsubstantiated as there is no proof that they only clean prior to inspections but a non-compliance is being issued as areas were found to not be clean, sanitary or well maintained

A complaint allegation was received alleging that child (RW) had a black eye but parents were not provided an incident report. RW stated another child was playing with a bike handle and hit her . A complaint inspection was conducted on 1/6/2024 and statements from the teachers (EL and GW) and office staff (NT)were received. Per the teacher (EL), sh does not recall the incident on the playground involving the bike and the child's eye. Teacher recalled and incident that occurred a few days prior where the child received a scratch. The incident report for the scratch was not provided to the parents until the following day. Per the other teacher (GW), she recalls the incident involving the other child and the bike. RW was sitting with the other teacher (EL) when the incident occurred. The other child rode up close to them and got off the bike. He let go the bike and the handles hit the child (RW) on the face. The child started crying and was taken to the front office to see the office staff (NT). Per Ms. NT, she did not see a mark on the child's face, therefore she informed the teacher to notify the parent at pickup time but to not write an incident report since no mark was observed. Complaint is Substantiated. A non compliance is being issued as parents were not provided with a written accident / incident report on the date of occurrence for an incident that happened while the child was in care
