Per DCF Handbook When napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician. Written documentation of a physicians authorization must be on file at the facility. Documentation must include the childs name, childs date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid. The infant room had two children in sleep sacks that restricted their arms. The sleep sacks were sent home, and parents were notified of the sack requirements. According to the owner, the facility will provide approved sleep sacks for infants

40-17 · Medication
Medication was not returned to the parent/guardian of the child listed on the supplemental. Per Provider a courtesy call was provided and the parent has not picked up the medication. Per Provider, she will drop the medication off at the parent's place of employment today. The Provider will ensure all medication is returned to the parent/guardian upon withdrawing from the program. Per DCF Handbook: E. Medical records in this section are the property of the custodial parent or legal guardian and must be returned to them when the child withdraws from the facility. The medical records are transferable if the child attends another facility
