41-01 · Immunization Records
During the time of the inspection, the Director advised the Licensing Specialist that there are currently 45 children enrolled, 10 of which had been newly enrolled since the last inspection. During the time of the inspection, the Licensing Specialist monitored 10 of the newly enrolled childrens records. During the time of inspection, Licensing Specialist observed 4 students that were missing immunization records. The Director was given until October 25th, 2024 to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.1 B Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations

During the inspection, Licensing Counselor observed on the playground the mulch under the fall zones were not 6 inches in depth you were able to see the ground. The director was given until July 26th, 2024, to come into compliance with this standard During the inspection, Licensing Counselor provided technical assistance, CCF Handbook Section 3.12.D.1- Outdoor Equipment. D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. Child Care Facility Handbook Page | 39 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Licensing Specialist observed in the four- and five-year-old classroom, there was one cup from home that did not have the last name listed on it. "3.9.5, B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily." A teacher wrote the last name on the cup; thus, returning this standard to compliance at the time of this inspection
