39-05 · Accident/ Incident Notification and Documentation
39-5: During the inspection, Licensing Specialist observed that that an accident report was created on 5/20/2025 for child 1 getting bit by another child. Licensing Specialist observed that there was no first aid documented on the actions taken section of the accident report on 5/20/2025. Teacher 5 stated that the director brought teacher 5 to the office and showed her how to write an accident report and the procedures for when a child gets bit. The director told teacher 5 that if a child gets bit, the bite must be cleaned, and ice must be applied. Then the teacher must notify the office and then the office will call the parents. Then the teacher writes the accident report. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs

30-06 · Diapering
30-6: During the inspection, Licensing Specialist observed that there were three diaper changing mats that were not impermeable, in that there was a tear in the material, exposing the foam, in the Infant classroom, Freshman A classroom and Freshman B classroom. During the inspection, the diaper changing mats were discarded. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.10.2, E When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use

28-02 · Bottles, Breastmilk, Formula and Infant Food
Licensing Specialist observed eight bottles from home that were not labeled with last name. "3.9.5, B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily." An office staff person wrote the last names on the bottles; thus, returning this standard to compliance at the time of this inspection

Licensing Specialist observed items, at tablet and a cell phone, were placed on the diaper changing table. "3.10.2, G. Items unrelated to diaper changing shall not be stored in the diaper changing area nor shall they be placed on the diaper changing table." The teacher removed the items; thus, returning this standard to compliance at the time of this inspection
