36-4: During the inspection, Licensing Specialist observed that there was only 1 employee with a valid CPR card with the skills stated. The director advised that there are 6 employees with a CPR card that did not state that the skills were met. Extra time is being provided due to the provider already having the CPR class with the skills scheduled on May 9 th , 2025. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 4.2.4, E CPR courses must include on- site, instructor- based skill assessments by a certified CPR instructor. Documentation of completion of the online course and on-site assessment must be maintained at the facility and available for review by the licensing authority

17-06 · Outdoor Play Area/Fencing
During the inspection, Licensing Specialist observed the fence in the far back of the playground had a gap of 7 1/2 inches which is more than 3 1/2 inches in between the poles and the metal fence. Licensing Specialist let the provider know to keep an eye on the gap to make sure it does not get any bigger. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.5 F- Outdoor Play Area. F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided

During the inspection, Licensing Specialist observed on the playground the mulch under the fall zones were 4 1/2 inches in depth not 6 inches in depth. The director was given until September 2, 2024, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.12.D.1- Outdoor Equipment. D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. Child Care Regulation observed that the facility stored harmful items, cleaning supplies, in a manner that allowed access by children in care, inside an unlocked cabinet. "3.2, B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times." Child Care Regulation observed that the toilet in the three and four year old classroom next to the kitchen was clogged. "3.7, I. Each toilet and sink must be maintained in good operating condition, cleaned and sanitized or disinfected as needed, but at least once per day. Sinks utilized for teeth brushing activities/procedures by the children must be sanitized prior to the activity occurring." Child Care Regulation observed that the two and three year old classroom, behind the infant room, did not have a hand hygiene procedures poster that included situations or times posted. "3.10.1 Handwashing Child care personnel and children must wash their hands thoroughly with soap and running water, dry, and follow personal hygiene procedures for themselves and while assisting others. Examples of activities when handwashing is required include but are not limited to: before and after eating, immediately following outdoor play, after toileting, following the use of any cleaners or toxic chemicals, before and after administering medication, and during food preparation and snack distribution." Child Care Regulation observed that one staff did not have a completed employment history verification documented. "5.1, D. The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Child Care Regulation observed that the indoor play area was not inspected daily and documented for November and December 2022. "3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months." 23-06 · Fire Drills
During the inspection, the facility had documentation of having an approved fire inspection completed on 11/21/2022. The last fire drill was conducted on October 11, 2022. The fire extinguishers are certified through October 2023. Child Care Regulation observed that the facility did not have documentation of a fire drill in November or December 2022. The owner informed that the facility did not do a fire drill in November 2022, nor did they replace it with an Emergency Preparedness drill. The owner informed that the fire drill was conducted in December and provided paperwork from the fire department, as they did the drill in response to the alarm going off due to technical issues.
