Licensing Specialist observed on video an infant falling twice backward on two separate occasions and falling forward in the crib on one separate occasion, hitting their head twice, falling backward, and hitting their lip on the crib, falling forward within arm's length of the teacher, and the teacher not seeing the infant fall. Provided technical assistance to the Director, stating that children must always be in sight and sound of teachers regarding supervision
