The facility failed to report the incident that occurred on February 7, 2025, a child escaped supervision while her three-year-old class was on the ballfield and slipped through the gate of the ballfield. She ran into the school and down the hallway to the entry of the YMCA which resulted in injury to the teacher who was chasing her

On 3/22/2023, An infant while being changed on the infant changing table fell off the infant changing table and fell to the floor. The child was airlifted to the hospital . The facility has a camera system which captured the entire event on video which was observed and reviewed
