At the tie of inspection licensing specialist observed and photographed a step 2 small sliding board that was cracked, and duct tape was used to repair the slide. Staff removed the broken slide from the playground and disposed of the equipment bringing this back into compliance. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 A.- General Health and Safety Requirements. A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children

23-23 · Fire Drills
AT the time of inspection licensing specialist observed and photographed the emergency exit blocked by a 2x4 board keeping the door closed. The door could not be opened from the inside. Licensing specialist had to go outside and around the building to the door and removed the board to allow for the emergency door to be opened in the event of an emergency evacuation. During the hours of operation the emergency doors must be able to be opened. TA The exits must be clearly marked, identifying the path to safety in case of an emergency, at all times during the operation of the child care facility. The exits must not be blocked at any time

At time of inspection, licensing specialist observed that the toilet in the staff restroom was not fixed. The director stated that she has a person to install the new toilet and they will have it installed within 30 days. All other issues noted in the previous inspection have been resolved including the storage of the nap items and a covering of the stored items in the restroom are now covered with a shower curtain. At time of inspection, counselor observed that the storage shelves in the teacher's restroom were not covered. Be sure to add a cover or store lose items in something non-permeable. The nap items are stored in plastic bags which are non-permeable. The toilet in the teacher's restroom did not have a tank cover and was not in good working order. Teacher stated that a new toilet has been ordered and just waiting for it to be installed. Be sure to store other lost items stored in the restroom in something non-permeable by due date. Also at time of inspection, counselor observed and photographed areas which needed cleaning. The center has an air purifier however the purifier has significant dust built up. The children's restroom has a stain in the bowl and a strong smell of urine was present. While completing the reinspection, the counselor observed an infant with a blanket in the crib. Per the CCF handbook 3.6.4.C. Children must not be placed in the cribs, playpens, play yards or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. Cribs, playpens, play yards, other napping and sleeping bedding being used by a child must be placed a minimum of 18 inches away from window blinds, draperies or any window treatment/cover that pose a strangulation hazard. Child Care Licensing observed the ratio of 1:5 in the infant/1yr/2yr old classroom. There were four one/two-year-olds and one infant with one child care personnel. The children were napping during this observation. TA provided regarding mixed age groups, explained that the inclusion of the infant with the group requires the infant ratio of 1:4.
