During the course of a complaint inspection, the following was found out of compliance; The child fell on the carpet during circle time and suffered a cut on his inner lip when his mouth hit the floor. Ms. VO spoke to the mom personally about the incident but did not write nor issue an incident report. the teacher stated that she didn't write an incident report because it happed near the end of the day and he was about to go home. TA was provided on accident / incident reporting requirements
