Per interview with childcare personnel, Teacher 2 was outside with the children while Teacher 1 was in the home. Teacher 2, who was outside supervising children came into the home to change a diaper which resulted in the children being outside alone. Teacher 1, who was in the home looked up and saw two children fighting and ran outside to separate the children. Teacher 2 returned outside with the child after diapering. Per childcare personnel the children were left outside alone for 3-5 minutes

Teacher 1 stated she didn't know the child was scratched until the child's parent asked her about the scratches on the child. The Provider will ensure accident/incident reports are written the day the accident/incident occurs and signed by the parent or the individual authorized to pick the child up. Per DCF Handbook: D. All accidents, incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child, and observed health related signs and symptoms, which occur at a home, on field trips or during transportation, must be documented on the day they occur. Documentation shall include the name of the affected party, date and time of occurrence, description of occurrence, actions taken, and signature of operator and custodial parent or legal guardian. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence

A child obtained scratches on the neck while in childcare. Childcare personnel will complete the Child Safety and Prevention course on the DCF Training portal. Teacher 1 and Teacher 2 will submit their DCF training transcripts to the licensing office on or before 03/14/2025 to the licensing office to verify compliance. Counselor arrived at the program and children were on the outside playground. The employee was inside with an infant. She answered the door and went to get the owner. The owner came inside, with employee, to let counselor into the home, leaving children on the playground with no supervision. Counselor explained to the owner that herself or employee must be outside at all times with children. When counselor was leaving, employee was still inside with infant, and owner walked inside to front door with counselor. Counselor advised again that owner or employee must be outside at all times with children. Owner returned outside. This re-inspection was conducted as a follow up to violation on 10/20/22 re-inspection report. See counselor comments for details
