17-02 · Outdoor Play Area/Fencing
Outdoor play area was missing sand box covers(two). Wasp nest was discovered on the left wing of playground, on the wood at the top. Adult metal shovel was found on the left side of playground. (director removed the shovel at the time of inspection) Large plastic container lid was broken laying on side. Playground equipment on left side needs to be power washed. Counselor has pictures on request. PER DCF HANDBOOK: 3.12 Outdoor Equipment A. A child care facility must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities. B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. C. All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. E. Equipment used for climbing should not be placed over, or immediately next to, hard surfaces such as asphalt, concrete, dirt, grass, or flooring covered by carpet or gym mats not intended for use as surfacing for climbing equipment. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. Any open containers with water must be emptied immediately after use, i.e. pots, toys, or other equipment that collects water. G. Sandboxes must be covered at the end of each day. The covering used must prevent access to the sandbox by animals. H. Wading pools, including inflatable water slides with a landing area where water collects, are prohibited. I. The provider shall maintain manufacturers instructions on file electronically or in paper format for outdoor playground equipment purchased or installed on or after January 1, 2020, and/or have available for licensing to review upon request. Corrective Action: Director will put a ticket into corporate. Director removed the shovel while counselor was there, then was complaint. Facility has until 1.2.25 to be in compliance

08-04 · Seat Belts/Child Restraints
Van has one seat belt that is broken. Second row behind the driver, next to the aisle. Counselor has pictures on request. PER DCF HANDBOOK: 2.5.5 Seat Belt/Child Restraints Each child, when transported, must be seated in a back seat in an individual factory installed seat belt or federally approved child safety restraint. The child safety restraint must be installed, secured and used in accordance with the manufacturers instructions and a copy of such instructions must be maintained (in the vehicle and/or on file). Child safety restraint must be replaced if they have been recalled, are past the manufacturers date of use expiration date, or have Child Care Facility Handbook Page | 18 been involved in a crash that meets the U.S. Department of Transportation crash severity criteria or the manufacturers criteria for replacement of restraints after a crash, as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by in 65C. Corrective action: Director put tape on that seat stating the seat belt was broken. Director placed a ticket to corporate per email for the seat belt to be fixed. 20-06 · Crib Requirements
Staff member put infant in crib with bib still around the neck. Second staff member removed the bib from around the infant's neck while still in the crib. Facility will retrain the staff on safe sleep and recommend to the director to inspect the bibs at the facility on a regular basis. Facility is having a staff professional day on June 7th,2024. At this time the director will retrain the staff on safe sleep. PER DCF handbook: 3.6.4 Safe Sleep C. Children must not be placed in the cribs, playpens, play yards or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. Cribs, playpens, play yards, other napping and sleeping bedding being used by a child must be placed a minimum of 18 inches away from window blinds, draperies or any window treatment/cover that pose a strangulation hazard

28-02 · Bottles, Breastmilk, Formula and Infant Food
The infants listed on the supplemental form bottles were not labeled with the child's first and last name. The Provider will ensure bottles are labeled with the child's first and last name prior to being stored in the refrigerator. The Provider wrote the infant's last name on the bottles at the time of inspection. Per DCF Handbook: B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily

The employee listed on the supplemental form did not complete the required training within the allotted time after returning to the childcare industry. Per Provider the employee is scheduled to take the required exams. The Provider will send the employee's DCF training transcript to the licensing on or before 09/18/2023 to verify training requirements are in compliance

04-04 · Supervision
On 12/22/2022, Child A was inadvertently left on the playground by Staff A without adult supervision for an unknown length of time. According to witnesses, the length of time was somewhere between 1-10 minutes. The violation was corrected immediately when the child was found safe and unharmed by Staff B, and was brought back into the building upon discovery. Staff A was placed on leave and later terminated. Additionally, the facility has since developed and trained all staff on a Playground Transition Policy and Name to Face Process, as verified by documents received in the licensing office via email, 1/23/2023. Facility was compliant with supervision requirements during the on-site complaint investigation
