To maintain an active Director Credential at either level, candidates must complete the renewal section of the CF-FSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which may be obtained from the Departments website at www.myflfamilies.com/childcare. B. A Director Credential renewal, as documented on the Departments training transcript is active for five years from the date of issuance. 03-01 · Ratio Sufficient
During the inspection, there were 30 children in care in the group that was using the large back playground. The children in this group were 4 yr olds - school age, with the majority being school age. A ratio of 25 children to 1 staff member was required for this age group. One teacher brought some children inside the building to use the bathroom, and was sending them back outside as soon as they finished. At one point, all but 2 of the children had returned to the playground with the one teacher who was standing on the porch, and the ratio on the playground was 1:28 for about 5 minutes, as observed by the licensing agent and other Department staff. Ratio was restored when the director was made aware of the issue and came to stand on the playground porch as well, until the other teacher returned. Recommend: Staff must be aware of ratio requirements at all times. Ensure that children who need to use the bathroom be taken into the building either by an additional staff member or in group sizes that ensure that neither teacher is ever left alone with too many children, to maintain ratio properly at all times. 04-02 · Supervision
On 4/10/2023, Teacher A was solely responsible for a group of children on the large, back playground. Rather than actively supervising the children, patrolling the playground and monitoring the play and activities, Teacher A was observed to be under the covered porch by the building and on her phone. Later that night, it was discovered that Child A had been attacked by Children B and C behind the colorful, big climbing equipment, located about 75 feet from the covered porch area. The attack was extremely aggressive in nature and resulted in physical and emotional damage to the child. Because Teacher A was not actively supervising the children, she was not aware of the incident on the day it occurred. Although the facility had been made aware of the incident the next morning, the licensing agent and other Department staff arrived on 4/11/2023 to find the same class on the same playground, and the attending teacher was also under the covered porch, not actively monitoring the children in her care. The children were observed this day to be engaged in extremely rough play, particularly in the area behind the colorful, big climbing equipment. Behaviors observed were shoving, yelling, tussling, etc.the children ceased their activities and disbursed as DCF staff approached the area, but as soon as they walked away, the children re-congregated and resumed their rough play. The licensing agent discussed the concerns and observations with the Director, who later discontinued the use of the playground space. The space was still not in use during the next few visits by licensing staff. For Corrective Action: Review the Child Care Licensing Handbook thoroughly for all sections discussing supervision, particularly Section 2.4.1, A, B, D, G, H, and I; 2.4.2; 2.4.3; 2.4.5, D. By the deadline, please hold a staff meeting to train staff on the importance of active supervision. Submit copies of the training agenda and the staff sign-in sheet to the licensing office as proof this has been completed

The Provider will ensure all required food groups are provided during breakfast, lunch, and snack. The Provider will ensure food provided by the childcare facility and food brought from home meets food requirements for childcare. Per Provider, food requirements are printed and posted on the cabinet. The Provider will discuss food requirements with all childcare personnel. Licensing counselor observed a child in the three year old class eating a whole hotdog and bun during lunch. Licensing counselor discussed food requirements with childcare personnel. Childcare personnel cut the hotdog into pieces for the child during the inspection. Per DCF Handbook: C. Foods that are associated with young childrens choking incidents must not be served to children under 4 years of age, such as, but not limited to, whole/round hot dogs, popcorn, chips, pretzel nuggets, whole grapes, nuts, cheese cubes/sticks and any food that is of similar shape and size of the trachea/windpipe. Food for infants must be cut into pieces inch or smaller, food for toddlers must be cut into pieces inch or smaller to prevent choking. This applies to all food, even food provided by parents/guardians

35-01 · Communicable Disease Control
Two childcare personnel had confirmed cases of conjunctivitis while working with children and did not isolate per medical recommendations of onset. Per childcare personnel antibiotics have been started to resolve the issue. Per DCF Handbook: B. Any child, child care personnel, or other person in the child care facility suspected of having a communicable disease must be removed from the program or placed in an isolation area until removed. Such person may not return without medical authorization, or until the signs and symptoms of the disease are no longer present. If the local health department official or primary health care provider suspects that a child or child care personnel are contributing to the transmission of the illness, are not adequately immunized when there is an outbreak of a vaccine-preventable disease, or the circulating pathogen poses an increased risk to the individual. The child or child care personnel must not return until the health department or primary health care provider determines the risk of transmission is no longer present. Child care personnel who work in the food preparation area may not return until the signs and symptoms of the disease have not been present for 48 hours
