Licensing Specialist observed a classroom with 6 children (ages: infant, 2, 3 and 4 year olds) and no teacher. A teacher was seen supervising the second group of children which consisted of 4 children (ages 3-4 years) while the teacher was carrying an infant. This standard was brought into compliance once the director arrived. 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. When asked the teacher she stated that she was watching both classes (one class containing 3 children and the other containing 6) . Licensing Specialist observed the screws of the handle of ramp of the playgroup very loose. The screws were coming in and out of the equipment. 3.12 Outdoor Equipment B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. FSC noticed the playground ground to have been with dirt along with a lot of leaves all over the ground that have not been picked up. Along with numerous poop on the ground. TA . All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

17-06 · Outdoor Play Area/Fencing
FSC observed a few gaps more than 3.5 inches along the fence line, along with the gaps between the gates. TA The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3.5 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided

FSC noticed two bottles of the children in a closet non refrigerated, in an area where there's a lot of dust. Along with that the children's bottles had no bottle caps on them in a closet.
