03-01 · Technical assistance was provided that The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. Staff that walked in from playground complied with ratio requirements and standard returned to compliance at time of inspection. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. Upon arrival to the facility, licensing specialist began to count ratios in the classrooms. When walking to the classrooms, licensing specialist observed staff rushing in from the playground area to classrooms. When entering a 1-year old classroom, a staff walked in along with licensing specialist. Licensing specialist had observed that same staff entering the 4-years old classroom when licensing specialist entered the infants classroom. That class had 9 children with one staff already in the classroom, plus the one that walked in. In another 1- year old classroom, there were 9 children with two staff, however one had just walked in as she was one of the staff that walked in from the playground

20-01 · Technical assistance was provided that no double or multi-deck cribs, cots, or beds may be used. Children up to one year of age must be in a crib or playpen/play yard with sides for napping. Crib sides must be secured while there is an infant in the crib. Bar spacing must not exceed two and three-eighths inches. Cribs or playpen/play yards must meet the regulations as outlined in Title 16, Parts 1219, 1220 & 1221 Code of Federal Regulations, 2014, which is incorporated by reference in 65C-22.001(7)(w), F.A.C. Provider informed that child is in transition to sleep on mat. The director brought in a playpen and moved the child to a playpen and standard returned to compliance at time of inspection

20-04 · Technical assistance was provided that when napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician. Written documentation of a physicians authorization must be on file at the facility. Documentation must include the childs name, childs date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid. FSC informed director that in order for child to sleep on tummy, the parent must provide physician authorization documentation. The staff turned the child to his back and called parent requesting physician authorization documentation

33-01 · Technical assistance was provided that child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel. hild care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. Part I Courses (30 Hours) Child care personnel must complete all of the following: ? Child Care Facility Rules and Regulations ? Health, Safety and Nutrition ?
