License Specialist observed that the Provider did not have documented proof of an annual fire inspection by the local fire authority. Technical Assistance was given to the Provider as per the SACCF Handbook that ( Unless statutorily exempted, all school-age child care programs must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Fire Safety Standards for Nonresidential Child Care Facilities, and must be inspected annually. A copy of the current and approved annual fire inspection report by a certified fire inspector must be on file with the licensing authority. Licensing Specialist observed that Three (3) Child Care Personnel did not complete the 40 hour Introductory Child Care Training requirement. Technical Assistance was given to the Provider as per the CCF Handbook that (Child care personnel, including volunteers that work 10 hours or more per month, must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. A. Part I Courses (28 hours) School-Age child care personnel must complete all of the following: ? Child Care Facility Rules and Regulation (6 hours); ? Health, Safety, and Nutrition (8 hours); ? Identifying and Reporting Child Abuse and Neglect (4 hours); ? Understanding Developmentally Appropriate Practices (5 hours) ? School Age-Appropriate Practices (5 hours) B. Part II Courses (12 hours) The remaining 12 hours must be met by successfully completing one of the training options identified below: ? The Departments school-age specific training and the competency examination offered by the Department or its designated representative with a weighted score of 70 or better. ? Completion of specialized school-age training, provided by a vocational-technical center, community college, college or university, national organization or affiliates of a national organization, that requires demonstration of competencies through passage of examination (s), or completion and assessment of a Professional Resource File (portfolio of materials that demonstrate competency). 12-02 · Technical assistance was given to the Provider as Per SACCF Handbook that ( All school-age program facilities must be clean, in good repair, and free from health and safety hazards and vermin. During the hours that the program is in operation, no portion of the building can be used for any activity that presents a risk to the health and safety of the children. Indoor and outdoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Any problems must be corrected before the area is used by children. License Specialist observed various areas of the facility not to be in good repair. 1. Room # 3, also known as the Percussion Room was missing the doorknob, leaving a 4.5" X 4.5" hole with splinters and sharp edges accessible to the children in care. 2. Room # 4, also known as the Cello Room was missing the left door vent, leaving a large hole on the door with splinters and sharp edges accessible to the children in care. 3. Boy's bathroom had a broken wall mount toilet paper holder leaving a sharp metal bracket exposed and a hole to the left in the ceramic tile with sharp edges accessible to the children in care. 4. The sink in the Girl's bathroom has broken porcelain surrounding the Grid Drain Strainer assembly leaving exposed broken sharp edges in Three (3) different areas of the sink accessible to the children in care. 5. Ceiling in the Staff Bathroom has Four (4) holes in the ceiling as recent lamp fixtures were repaired
