12-02 · Facility Environment
License Specialist observed that various items in the children's bathroom were not in good repair. 1. The children's sink was missing the right panel (end /finished, formica panel) exposing the unfinished rough plywood, chaff wood material, splinters and rough edges accessible to the children in care. 2. Due to the missing Right, side panel of the sink, there is a GAP (separation) of Two (2) inches in approximation consisting of Two (2) broken floor tiles. The tiles contain sharp edges with porcelain splinters and a curved jagged edge against the wall accessible to the children in care. Technical Assistance was given to the Director as per the CCF Handbook that ( All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. )

License Specialist observed that in the 3/4 Year Old Classroom there was an electrical outlet without cover accessible to the children in care. Technical Assistance was given to the Director as per the CCF Handbook that ( All accessible electrical outlets must be tamper-resistant electrical outlets that contain internal shutter mechanisms to prevent children from sticking objects into receptacles. 13-03 · Toxic Substances, Hazardous Materials and Weapons
License Specialist observed that the fish tank cabinet located in the 3/4 Year old classroom at the entrance of the facility contained harmful items including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials were accessible to children in care. These items were on the bottom shelf all piled together on an electrical extension included with a plugged and active Timer for the fish tank. Items in the shelf included AQUA CARE aquarium water treatment chemical solution, magnetic cleaner, Two (2) containers of Tetra Min Tropical Flakes - fish food, chemical measuring strips, fish tank power supply, instruction booklets and other multiple items. Technical Assistance was given to the Director as per the CCF Handbook that ( All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times

License Specialist Observed that the facilitys outdoor play area contained litter, nails, glass or other hazards that posed a low potential for harm to children. The observation consisted of the gate which enclosed the centrifugal electric water pump was open and without a lock allowing accessibility to the children in care. 17-07 · Outdoor Play Area/Fencing
Licensing Specialist observed that the base of the fence in the outdoor play area on the right side, street perimeter was not at ground level and could allow inside or outside access by children or animals. Technical Assistance was given to the Director as per the CCF Handbook that ( The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area.The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided

23-09 · Fire Drills
License Specialist observed that the facility did not maintain fire drill records for the months of operation for a minimum of 12 months from the date of the fire drill. LS reviewed fire drill log and the recorded fire drills were completed between the dates of January 23/2024 to May 22, 2024. Director stated that she had the prior completed form but she was not able to provide it during the time of the inspection. Technical Assistance was given to the Director as per the CCF Handbook that ( A current attendance record and parent contact information must accompany child care personnel out of the building during a drill or actual evacuation and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. One fire drill using an alternate evacuation route, 2. One fire drill during napping/sleeping times, and 3. One fire drill in the presence and at the request of the licensing authority. This drill will be coordinated with the operator or designee. License Specialist observed that a complete CF-FSP Form 5131, Background Screening and Personnel File Requirements, was not on file for Two (2) Child Care Personnel. 33-01 · Technical assistance was provided that Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility.
