During the inspection the Family Service Counselor observed that the Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age- appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide Child Care Facility Handbook Page | 60 documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. This violation was resolved in the presences of the family service counselor

During the inspection the Family Service Counselor observed that three of the students at the facility did not maintain documentation that the parent(s) or legal guardian(s) of each child were provided information regarding the potential for distracted adults to fail to drop off a child and leave them in the car annually during the months of April and September in that [ the provider didnt given the document to the parents in person or online for the month of April 2024 for the family service to review during the renewal inspection on 5/10/24]. Technical support that was given to the provider stated that the annually, in the months of April and September, the child care facility must provide parents/guardians of the children enrolled in the facility with information regarding the potential for distracted adults to fail to drop off a child at the facility and instead leave them in the adults vehicle upon arrival at the adults destination.CF/PI 175- 12, brochure, which is incorporated by reference in 65C-22.001(7)(x), F.A.C. and may be obtained from the Departments website at www.myflfamilies.com

20-06 · Crib Requirements
FSC observed 3 infants sleeping on a crib, each infant was swaddled in a white blanket, additionally one infant had a bib around his/her neck. Infant Teacher stated that the child was placed with a blanket because it's too cold in the room and that one infant is teething and is drooling and has a rash on his/her chest but did not have a written authorization by a Dr. that allows the alternate position or items. As Technical Assistance, per Rule Handbook: Safe Sleep B. Sleep sacks that fit according to manufacturer's recommendations, do not restrict the infants' arms, and will not slide up around the infant's face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the child's physician. Written documentation of a physician's authorization must be on file at the facility. Documentation must include the child's name, child's date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid. C. Children must not be placed in the cribs, playpens, play yards or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. Cribs, playpens, play yards, other napping and sleeping bedding being used by a child must be placed a minimum of 18 inches away from window blinds, draperies or any window treatment/cover that pose a strangulation hazard. As a solution, One of the childcare personnel removed each infant from the blanket, removed the bib from the infants' neck and placed each infant back to sleep on his/her back

FSC observed an infant sleeping on a crib, swaddled in a blanket, there were 2 round pillows on the crib that helped positioned the infant on his/her right-hand-side. Infant Teacher stated that the child was placed on that position because the infants suffers from reflux but did not have a written authorization by a Dr. that allows the alternate position.
