Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. Gave TA during Complaint inspection 3/19/2025, specialist observed that fall zone is not in compliance. TA was given to provider for new fall zone to be redone, as it is a safety hazard

During a walkthrough of the facility, LS observed that the 3-year-old room, which is licensed for a capacity of 12 children, was housing 14 children exceeding the allowed capacity by two. 12-07 · Facility Environment
LS observed a safety deficiency having a moderate potential for harm to the children in care was observed. The TV is placed on top of the dresser like where the children stand in front of the TV. If the child moved the stand or pushed the TV it can easily fall on the children. TA Equipment, materials, furnishings, and play areas should be sturdy, safe, and in good repair. Provider should monitor the Consumer Product Safety Commission (CPSC) recommendations for use of equipment. Walls, ceilings, floors, furnishings, equipment, toys, and other surfaces should be suitable to the location and the users. They should be maintained in good repair, free from visible soil and clean. Equipment and furnishings must be arranged to allow direct supervision of children by child care personnel from all areas within a room/play area. Bookshelves, televisions, and other heavy furnishings must be stable and secured as to not pose a threat of falling over. In efforts to prevent children from climbing to dangerous heights or enabling them to reach hazardous items, chairs and other furnishings must be placed away from cabinets and shelves. The program should make accommodations to the program environment and schedule so that children with special needs may participate
