03-01 · Technical assistance was provided that The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. The 2-year old teacher that was in break returned shortly after FSC arrived to facility and the office staff returned to 2-year old classroom where there are still 1-year old children at the end of the inspection. 1-year old child in infant classroom was transferred to 1-year old class and the standard returned to compliance at time of inspection. Upon arrival to facility, FSC counted ratio. FSC observed one staff with 5 children (4 infants and 1 one year old). Provider stated that child was in infants classroom because she not yet walks, but is already one-year old. Child was transferred over to 1-year old class. When continuing with ratio, office staff informed FSC that regular teacher for 2-year old classroom was in break. Office staff was covering in that class until teacher arrived from break. Upon reviewing children files, FSC found 4 of the children from the 2-years old class are still 1-year old. 42-02 · Technical assistance was provided that The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B.
