03-01 · Technical assistance was provided that the staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. Staff in charge at time of inspection said one staff was on break and another staff had gone to a doctor's appointment and would return. One staff returned at 1:14PM and went to the 1-year old classroom to meet ratio requirement. The second staff did not return while FSC was present. Two children from the 2-year old classroom were moved to the 3-year old classroom to meet ratio requirements. 42-02 · Technical assistance was provided that The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B.
