FSC observed that the facility failed to provide the parent with an incident report the day of incident. Provider was notified by another member of the agency that this form is required to be provided to parent. Provider was able to correct at the time of inspection and technical assistant was given to provider per CCF handbook " B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. "

Technical assistance was given to provider per CCF handbook " Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. "
