Licensing Specialist when reviewing the childcare personnel file noticed that the CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form does not sign annually. Per provider, the childcare personnel signed and dated the Child Abuse and Neglect Reporting Requirements form at the time of the inspection. T.A. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. All (13) child care personnel records in the system were reviewed of which (13) were updated for compliance. 5 new personnel were hired since the last inspection as per the director
