LS observed a platform used by children was not, safely constructed with an impermeable surface that can be easily cleaned and sanitized or disinfected. Toilets in the center in the restroom for the children to use, did not have the toilet seat on them. Along with the toilets being in the open and no stalls/ doors for privacy for the children. LS observed childcare personnel failed to share the accident/incident form with the custodial parent/ legal guardian or the individual authorized to pick up child on the day that the incident occurred.
