Licensing Specialist observed that the facility's outdoor play areas continued that contained littler, nails, glass and other hazards such as tripping hazards that posed a low potential for harm to children in care continued NOT to be corrected specifically #4 under comment section (protruding tree roots growing on the outer fence and uneven pavement.) License Specialist observed that it was in the same state as previous Re-Inspection conducted on 12/12/2024. Technical Assistance was given to the Provider once again as per the CCF Handbook that ( The outdoor play area must be clean and free from litter, nails, glass and other hazards.) Also, to continue standard in compliance, maintenance of Playground area for protruding tree roots growing on outer fence and uneven pavement must be kept and maintained to avoid any tripping hazards.) Closet contained: mops, bleach, cleaning supplies, degreaser, brooms, multiple glass vases, stain remover liquid, floor cleaner, stainless steel spray cleaners and other household/industrial cleaning items. License Specialist observed that the play equipment was not maintained in a safe or sanitary condition for the children to use such as: 1. Three (3) Water/Sand tables throughout the playground were broken with sharp edges in several areas, they were unclean and unkept. Toddler, beige sand/water table contained water and sand together with multiple toys floating inside. 2. Blue and Yellow playset ( GAP Play system) was not maintained in sanitary conditions. 3. Several balls, basketball (blue and brown) were broken and deflated. 4. Two (2) Red Tricycles were missing pedals, and several silver tricycles were missing handlebar covers, exposing the rusted sharp edges to the children in care (some had tape) but it was wearing out at the time of inspection. Wheels and body of the tricycles were rusted and unkept. Technical Assistance was given to the Provider as per the CCF Handbook that (All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. Any open containers with water must be emptied immediately after use, i.e. pots, toys, or other equipment that collects water.)
