Technical support that was given to the provider stated that the Student Health Examination or the signed statement is valid for two years from the date the physical was performed. During the inspection the counselor observed that the facility's first aid kit was missing the following items: [ Bottled water]. Technical support that was given to the provider stated each kit must be kept in a closed container and labeled First Aid and must, at a minimum, include: 1. Soap and hand sanitizer (to be used with supervision if hands are not visibly soiled and if no water is present), 2. Adhesive bandages, 3. Disposable, non-porous gloves, 4. Cotton balls or applicators, 5. Sterile gauze pads or rolls, 6. Adhesive tape, 7. Digital thermometer, 8. Tweezers, 9. Pre-moistened wipes, 10. Scissors, 11. Bottled water (for cleaning wounds or eyes), and 12. A current resource guide on first aid and CPR procedures. This issue was resolved during the routine inspection onsite in the presences of the counselor

33-01 · Technical assistance was given to provider per CCF handbook " Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A.
