42-02 · Technical assistance was provided that The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program

03-01 · Technical assistance was provided that the staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. Upon arrival to the facility, FSC observed 3 staff caring for 15 children in the infants and one-year old classroom. Per provider, the classroom is divided into two classrooms. However, FSC observed no separation, and the classroom has a capacity for just one classroom (capacity is 23.61). 33-01 · Technical assistance was provided that child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. 23-02 · Technical assistance was provided that unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A-36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. If the program is granted a fire inspection exemption by the local fire inspection office, the exemption must be documented and maintained on file at the program. Last fire drill was conducted on 4/28/2023. One fire drill using an alternate evacuation route was done on 3/31/2023. One fire drill during napping/sleeping times was done on 1/27/2023. One fire drill in the presence and at the request of the licensing authority was done on 5/30/2023. One emergency LOCKDOWN drill has not been conducted during licensure year. One emergency INCLEMENT WEATHER drill was done on 9/30/2022. Fire inspection conducted on 5/15/2023. Fire extinguisher serviced on 12/2022. As technical assistance, the licensing counselor requested that a Fire Drill be done in the presence of licensing authority, and the provider agreed to conduct the fire drill at time of inspection
FSC reviewed fire inspection dated 3/8/2023 which has results of "Final Notice" due to a outstanding violation for change of occupancy. FSC requested letter of DCF Day Care Change of Use from fire department and they provided a more recent fire inspection from 5/15/2023 with additional violations in the report. DCF Day Care Change of Use letter was received after leaving site. FSC contacted provider and advised about the most current fire inspection with outstanding violations. Provider is awaiting a re-inspection and will e-mail new report once completed
