FSC observed the Director to have an expired Director's Credential. Per DCF Handbook, "Each childcare facility must have a credentialed director who is on-site a majority of hours, excluding weekends and evening hours that the facility is in operation. Documentation of majority of hours must be maintained and available for review by the licensing authority. B. CF-FSP Form 5252, Florida Child Care and Education Program Director Credential, must be maintained at the facility for review by the licensing authority. A Director Credential, as documented on CF-FSP Form 5252, Florida Child Care and Education Program Director Credential, is active for five years from the date of issuance."

41-01 · Immunization Records

FSC has noticed when reviewing the children's records that several of them do not have up-to-date immunization records. TA was given to the provider: (If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations)

FSC observed a classroom of 17 18mth old children with 2 teachers. When the third teacher came back in the classroom the ration was upheld. 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children, and applies at all times while children are in care. Non compliance was completed at time of inspection. 18mth old- 2year old classoom 2 teachers- 17 children. 1(6) 3-6 year old classroom 2 teachers- 25 children. 1 (15) 2 teachers- 21 children. 1(15) 2 teachers-25 children. 1 (15) 2 teachers- 17 children. 1(15) 2 teachers- 20 children, 1 (15)
